Other then the pins, jumpers is also needed.

An image of your io board will help solving this issue.

Super Mario World, Flickering in Caves

So, with levels below ground, with all its twinkling stars(?) I get flickering on the screen no and then, like a fast white flash. Samsung G5 32 inch.

Sync at 0. Using 1920x1440.

Is it my monitor or somthing else?

Did you try changing the display setting, in the mt32-pi menu?

Have you manually updated your mt32-pi? If so, there might be some settings in tech mt32-pi ini file that needs to be changed.

Also, can you post an image of the soldered pins, next to the io port, where you plug in your mt32-pi.

So I grabbed [MiSTerFPGA] Sharp X68000 Starter Pack when booting I get a blank screen. I notice the images are HDFs when the github asked for HDDs and FDDs. Does this matter? trying to play Tyrian, I'm experiencing slowdowns and speed up. Clouds slow down the game. watching the YouTube configuration video, there was no issue with the game.

Sadly not. It's just a game from my youth.

Have you saved the setting after restart. So it is mounted at next boot?
