I use a very similar setup except I output via optical to a DAC and then either pipe it through an amplifier or use my bluetooth transmitter with headphones.

Regarding lag, I've not noticed anything wildly out of sync but then I don't think I'm very sensitive to it.
It always seems to sound ok to me

I haven't used it for a while but there is a setting to save your config to cloud storage. Might have been dropbox
I used to use it but don't anymore so can't remember its name and I'm not near my MiSTer to check but it's worth having a look - might be the scripts folder I'm thinking of

To load the Dragon 32, you actually load the Coco2 core and then sure the core options to select which machine you'd like

Edit - Sorry, I re-read your post and realised that my answer is useless.
I would probably try removing the core and then running the update script to see if it appears

As far as I'm aware, shaders etc aren't available for ScummVM as it isn't actually a core but is launched directly on the ARM core as software. On top of that, I think it was done more as a passion project and as such isn't officially supported. Saying that, "officially supported" doesn't really apply to this as it's an open source projec...

TL:DR
If I had the funds available, my first thing to do would be replace the RAM module and see if it helps.
If I didn't have the funds, I'd probably just hang on a while and see what core changes occur in the future, one of them may inadvertently (or intentionally) fix the issue.

Here is a mention of the issue I remembered.
I don't have time to read through it all but there are several mentions of the 2.5 revision board having issues with the early Saturn core.

I could be barking up the wrong tree here but it might be worth checking your ini settings. You can change the output quality of the HDMI audio I believe.
