Micro SD cards have not ever proven to be reliable in the long term for an OS in my experience. I would pop it out and check it for errors if you can. Or maybe setup a spare card and test this there. I'm wondering, is it safe to swap controllers into your SNAC connector (the level shifter thing) while your MiSTer is on? I'm guessing it is but does anyone know? You should be okay as long as the Extension not too long. Yes the length of wire will drop voltage.
If you want to use a DC extension cord, the ones rated for CCTV Cameras work well for this purpose.

if you are worried, or if the DC extension don't work, you can try to use a extension cable from your outlet to your AC/DC converter instead.

It wouldn't be hard to ground the case. As there multiple places to tap onto ground, example screwing the brass stand-offs into the metal of the case is often enough to get a ground connection going. Nothing that I am aware of.

The MiSTer Menus are kinda stripped down for technical reasons, although I don't see why the option to hide or disable parts of menu can't be enabled.

You can go and not map the Home/guide button on the control pad. But that's the best I can think of at this time.

ARC is a separate issue from CEC
CEC is a "Networking feature" of HDMI while ARC is more an alternative to S/PDIF to transmit Audio.

A dental pick popping out a pin in a short HDMI extender or Male to Female coupler, or buying a remade adapter should eliminate the powering up issue entirely.

The vibration of the PS1/PS2 pads are from a 7 to 9 volt line. Nothing on the MiSTer supplies that.

As for the converter?
Perhaps your USB solution doesn't have enough amps
