My PC Engine CD images are from the Redump set. Just to make sure that nothing went wrong with the image, I went to Redump's website and compared the MD5 hashes of all the bin files for the game. I couldn't spot anything wrong with the bin files' hashes. The cue file's contents appear to be identical as well. I make no assumptions about downloads ...

I know the original print run of the PC Engine port of Golden Axe refuse to run on anything but a Systems Card version 1.0. A Later reprint fixed the issue, abet the actual game disc is rare. Can you give a specific example ? I haven't seen anything of this sort on any any rips made properly. However, I have seen a few video captures of things like this, and the reason was because the disc image of the game being played was bad. The Arcade Card is not possible to have connected at the same time as HuCards on real hardware, as they can't both fit in the cartridge port at the same time. I suspect that the issue you see has nothing to do with the Arcade Card (but may still be some other kind of bug); Sorgelig confirmed that Arcade Card activity is dependent on CDROM operation...

My understanding based on my studies of the CDs, is that the duplicate copy of the data track is for redundancy in case the "Track 2" version is somehow damaged. However, I don't believe that I've ever seen/heard/witnessed such a failover.

I see this too.
Some text pixels show as black (the picture in the bug report isn't clear on this), when on real hardware they show as the same color as the other pixels.
Furthermore, during the intro, the top line of the screen shows part of the sprite data (and real hardware doesn't).

I use Sandisk Ultra cards, and I've never had a problem.
