Placed an order last night for one of the acrylic cases and need to change from the digital IO to the analog IO version. It there a specific way you're supposed to use the alternate mounting kit? Not really, it is just an alternate standoff size for Raspberry Pi cases which use M2.5 sized threads. It sounds like your switch is faulty, just email me with your original order number and I will get you a replacement sent out. Even with the Meanwell 4A power supply, if you are powering the mt32-pi over the USB3 cable it will display a low voltage warning. I have seen similar issues with JAMMA type adapters including J-PACs, when using an IO board.
One solution I know which works is remove C6 on the underside of the IO board it's the 100uf on the SOG switch, and just jumper it with a wire.

Many of the Chinese produced v6.0 IO boards seem to have this issue, as in they are all missing the P7 header pins. How can I disable them? If you have the analog IO board v6.1 you can cut this trace on the underside of the circuit board, this will disable all three coloured status LED's to enable them again you will need to bridge the trace again. I've seen somewhere that the IO board needs an additional mod but so far have been unable to locate details for that. As you realised the v6.0 IO board will not work with the SEGA controller adapter, here is some information on the modification. Yes that is correct, the Atari adapter also requires an ADC input and a 3.5mm AUX cable if you wish to use the Atari paddle controller. I think your fan is possibly faulty as the Noctua NF-A4x10 5V should work perfectly using the 3.3 volt jumper selection, albeit at a lower rpm.
