I do get screen tearing at the bottom of the screen on my TV (and in every core) if I try to use vsync modes 1 or 2. I need to stick to 0 unfortunately so that there is no tearing. My TV doesn't jive well any signal that isn't exactly 60hz. Maybe that's what you're getting?

But i have on 1;

; Set to 1 for automatic HDMI VSync rate adjust to match original VSync.
; Set to 2 for low latency mode (single buffer).
; This option makes video butter smooth like on original emulated system.
; Adjusting is done by changing pixel clock. Not every display supports variable pixel clock.
; For proper adjusting and to reduce possible out of range pixel clock, use 60Hz HDMI video
; modes as a base even for 50Hz systems.
vsync_adjust=1

For me it happens a lot when using IO board VGA output to a CRT monitor. Re-loading the core, for some reason, usually fixes it.

Rarely happens on HDMI output.
