Mine is attached here, in case it's helpful.
Menu forward is bottom right, menu back is bottom left.
In one file, OSD Menu is mapped to pressing 1 and 9 simultaneously. In the other file, OSD Menu is not mapped so that you can use 1 and 9 to pause the game. I couldn't get the Menu mapped to another combination successfully.

Are you using the official Retronic firmware or the modified firmware? With an Intellivision controller some keypad combos will make the same signals as a disc and button combo, so only certain combos should be used i.e. should be from different rows and colums and seperated by one row. The official firmware doesn't support button combos that make unique signals other than keypad 1&9.

Those are with the modified firmware.

The modified firmware supports the keypad combos 4&E, 5&C, 5&E, the same as 1&9. Do those not work?

I got 5&E working now! It only works this way with the modified firmware. Otherwise you either have to use 1+9 for OSD or use a different controller.

Anyway, looks like the modified firmware is the way to go.

Here's my new input files.

These use the modified 3.3 firmware posted 1 page back, using an AT Games Flashback controller. 5+Enter for the OSD Menu. Bottom right for menu OK, bottom left for menu back. 1+9 works for pause.

I think this is the best it's going to get.

I'll add that it should be possible to add some debouncing code to the Retronic USB, to mitigate the "twitching" experienced with some Intellivision controllers.

I think I will need that code for the Flashback controllers I have. That twitch is enough to make play frustrating. I am appreciative of your efforts, but I am confused that it works for Ashfall but not for me.

When I contacted Retronic Design back in 2022 about this, this was their reply:

Intellivision flashback controllers are special beasts. There are two modes of operation in one. If you open the Windows calibration tool, you should see both a 16-direction joystick and a flock of buttons.

Buttons are representing two things; 16 single buttons on the controller and the 8 last buttons are representing raw reading of the controller itself.

You can try JzIntv emulator that was specifically using this scheme of reading with special button combination.

For the MISter, there is no specific support for this raw mode. Button mapping does not permit using this way of doing it.

I own a MISter and I did not try Intellivision controllers on it. I need to read how it’s built and if it’s possible to integrate a special controller like this.

Maybe the MISter team would be interested in integrating this to their base software.

The "raw mode" was added in Retronic firmware v3.1 or v3.2, I believe. So if you were using a Retronic USB with v3.0 you could get it setup and then update firmware to v3.3 and it should still work. Ashfall said he was using later firmware so I'm not sure what happened exactly.

The "raw mode" extra buttons makes each button press look like a three button combo. Some emulaters, MAME for example, can record a three button combo so it works anyway. Others like Bliss fail to interactively do the mapping. Jzintv has no interactive mapping so you can just ignore the extra buttons when editing the mapping file.
