How do you use Arcade Organizer?

I'm trying to figure out the best way to use Arcade Organizer. After disabling Skip MRA Alternatives I thought there would be one ROM for each core in the main Arcade folder and then all alternative ROMs would appear in the alternatives folder. That seems to be the case for some cores (Armed Police Batrider) but not for others (DoDonPachi).

What have you found to be an ideal config?
